Job Summary
Conduct home health physical therapy visits with adult and geriatric patients across Lakewood, Wheat Ridge, Arvada, Denver, and Aurora. Drive between patient homes and senior living facilities, completing a caseload of 3–7 visits daily. Perform OASIS documentation for all start-of-care patients and maintain electronic charting. Work full-time, Monday through Friday, with 35 visits per week. This stable, well-rated Medicare provider offers a generous pay range of $54–$57.70 per hour, including health insurance, HSA, dental, paid time off, and paid mileage. New grads are welcome.
Required Qualifications
- Bachelor's, Master's, or Doctorate in Physical Therapy
- Active PT license in the state
- US Citizen or Green Card
- COVID-19 vaccine
- Comfortable with 35 visits per week schedule
- Comfortable with service area covering Lakewood, Wheat Ridge, Arvada, Denver, and Aurora
- Comfortable traveling between patient homes
- Comfortable with OASIS documentation
