Home Health Physical Therapist (PT)

Provide physician-ordered physical therapy services in patients' homes, evaluating function and developing individualized plans of care. Conduct initial evaluations, reassessments, and plan-of-care updates every 30 days while supervising Physical Therapy Assistants and Home Health Aides. Educate patients and caregivers on exercises and home care routines, then collaborate with physicians and the interdisciplinary care team. Complete timely clinical documentation, including OASIS, and participate in quality initiatives, in-services, and care conferences. This full-time role involves seeing multiple patients per day in their homes or senior living communities with paid mileage reimbursement.
