Job Summary
Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients across South Bay San Diego, including Chula Vista, National City, and Imperial Beach. Implement therapeutic interventions focusing on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility. Schedule and manage your caseload based on availability while collaborating with a supportive care team to ensure coordinated patient outcomes. Utilize the integrated TherapySync EMR for compliant charting, with all administrative tasks such as referrals, authorizations, and invoicing handled by the platform.
Required Qualifications
- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
- Experience in home health PT or transition from outpatient or SNF settings

- Spanish and Tagalog language skills
