Provide in-home skilled Physical Therapy for geriatric patients in East County San Diego, conducting in-home evaluations, developing individualized plans of care, implementing functional interventions (gait training, balance, fall risk reduction), managing caseload with a collaborative care team, and documenting with TherapySync EMR. Requires valid California PT license, CPR/BLS, driver’s license, auto insurance, TB test within a year, and any applicable liability/business license.
Required Qualifications
