Job Summary
Home Health Physical Therapist (PT) delivering patient-centered, in-home care to geriatric patients in East County San Diego with flexible scheduling. Provide skilled PT evaluations, develop individualized plans of care, implement functional strengthening, gait training, balance improvement, and fall risk reduction. Manage your own caseload, schedule home visits per availability, and collaborate with a multidisciplinary team. Document using TherapySync EMR and coordinate with referrals, authorizations, and DME for streamlined administrative support. Role emphasizes autonomy, app-based workflow, and bi-monthly direct deposits.
Required Qualifications
- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
