Job Summary
Home Health Physical Therapist (PT) needed in East County San Diego (Alpine, El Cajon, La Mesa, Lakeside, Lemon Grove, Santee, Spring Valley, Campo, Descanso) to conduct in-home evaluations, develop individualized plans of care, and implement functional interventions focusing on gait, balance, and fall risk reduction. Role supports flexible, self-managed caseloads with app-based workflow, streamlined documentation via TherapySync, and coordination with an interdisciplinary team. Ideal candidates hold a California PT license, CPR/BLS certification, valid driver’s license with auto insurance, and current TB test; liability/business license if applicable. Per-visit pay structure with regional coverage in home-health settings offering autonomous scheduling and patient-centered care.
Required Qualifications
