Job Summary
Provide Skilled PT Care in-home for geriatric patients, including comprehensive evaluations, individualized plans of care, functional strengthening, gait training, balance, and fall risk reduction; manage a caseload with scheduling of home visits; collaborate with care teams; and document using TherapySync EMR. Independent in-home care with flexible hours and per-visit compensation in East County San Diego (El Cajon, La Mesa, Lakeside, Spring Valley, Santee, and surrounding areas).
Required Qualifications
- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical TB test (within the last year)
- Professional liability and business license (if applicable)
