Job Summary
Home Health Physical Therapist (PT) provides skilled in-home evaluations and individualized plans of care for geriatric patients, focusing on functional strengthening, gait training, balance, and fall-risk reduction. The role involves managing a personal caseload, scheduling/home visits per availability, collaborating with a care team, and documenting using TherapySync EMR. Independent practice with app-based workflow enabling flexible hours, referrals, authorizations, and administrative tasks handled centrally to maximize patient care. Licensure in California, CPR/BLS, valid driver’s license, andTB test within the past year are required. Primary service area includes East County San Diego (El Cajon, La Mesa, Lakeside, Lemon Grove, Santee, Spring Valley) and surrounding locales, with in-home patient care and no stated relocation requirement.
Required Qualifications
- California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
