Job Summary
Assess, plan, and implement rehabilitation programs to improve mobility, relieve pain, and enhance strength for home health patients; educate clients and family members on exercise programs and therapeutic procedures; document observations, interventions, and progress; evaluate and adjust treatment plans in collaboration with physicians and caregivers; perform patient care as prescribed, review referrals and medical records, and participate in educational activities and clinical studies; requires CT PT licensure and a DPT-level education with strong clinical assessment and communication skills; bilingual Spanish preferred; CT-based home health environment with potential for charting and computer documentation; mobility up to 50 pounds, travel involved.
Required Qualifications
- Graduation from an approved Physical Therapy curriculum
- Current and valid Connecticut Physical Therapist license (required)
- Driver’s License, Car Insurance, Car Registration
- Recent work experience in home health (preferred)
- Bilingual (Spanish) preferred
