Job Summary
Evaluate and treat patients with physical disabilities, injuries, or mobility limitations in a home care environment. Establish treatment plans utilizing exercises, stretching, manual therapy, and equipment to manage pain and increase mobility. Educate patients and family members on rehabilitative care and activities promoting independent living. Collaborate with other providers to ensure optimal patient outcomes. Requires current unrestricted PT licensure in the state, CPR/AED/BLS/First Aid certification, reliable transportation, and one year of home health experience. Full-time or part-time roles available with flexible scheduling.
