Home Health Physical Therapist (PT)

Deliver individualized physical therapy services, including evaluations and treatments, directly in the patient's home environment. Perform therapeutic interventions focused on functional strengthening, gait training, and fall risk reduction to help patients reach independence goals. Document patient responses and maintain accurate records using our integrated EMR system while managing your own visit cadence and schedule. This role supports a growing network serving geriatric patients across Tucson, Oro Valley, Marana, Sahuarita, Green Valley, and surrounding communities, with ultimate geographic flexibility to choose your preferred neighborhoods.
