Job Summary
Conduct initial and ongoing assessments of mobility, coordination, strength, and functional levels for patients in the Sebring FL area. Administer, evaluate, and interpret tests to define functional goals, while delivering physical therapy services that comply with physician orders. Maintain care plans through timely documentation, modify treatment as needed, and coordinate discharge planning with caregivers and the broader healthcare team. Supervise personnel including PTAs and HHAs where applicable. Complete OASIS assessments per state regulations and ensure documentation meets payer standards to prevent denials.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Full TIme Visits Mon - Friday for Sebring FL area
