Job Summary
Conduct initial and ongoing assessments of mobility, coordination, strength, and functional levels to determine goals and outcomes. Deliver physical therapy services per physician orders while maintaining comprehensive documentation of client performance and modifying treatment plans as needed. Provide written evaluations, discharge care plans, and education to patients and caretakers regarding home exercise regimens. Coordinate services with physicians, nurses, and other healthcare team members, and supervise PTAs and HHAs where applicable. Complete OASIS assessments and ensure documentation meets payer standards to prevent denials. Operate within the Indian River County territory on a Monday through Friday schedule with weekend on-call rotation.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Schedule is Mon - Friday with weekend rotation for On Call
- Territory: Indian River County
