Home Health Physical Therapist PT Full Time 7500 Bonus

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, progress, and expected outcomes. Maintain plans by documenting client performance, modifying goals, and determining discharge care plans. Collaborate with physicians, nurses, and caretakers for discharge planning and education on home exercise plans. Supervise personnel including PTAs and HHAs where applicable. Complete OASIS assessments per state regulations and document effectively to prevent clinical denials. Full-time schedule Monday through Friday with a $7,500 sign-on bonus. Requires a Physical Therapy degree and current unrestricted state license.
