Job Summary
Conduct initial and ongoing mobility assessments to evaluate strength, coordination, and functional levels. Deliver physical therapy services per physician orders while administering evaluations to define goals and outcomes. Maintain discharge plans by documenting client performance, modifying treatment, and educating caretakers on home exercise routines. Collaborate with physicians, nurses, and other team members to coordinate care and supervise personnel as applicable. Complete OASIS assessments and ensure documentation accuracy to prevent technical denials.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
