Home Health Physical Therapist (PT) - Contingent

Provide skilled physical therapy in accordance with the physician's plan of care, accepting only patients referred by the patient care manager. Assist in assessing functional levels through appropriate tests, develop and revise the plan of care including OASIS assessments, and treat patients to relieve pain and restore function using specified modalities. Observe, record, and report patient reactions and condition changes to the physician and staff. Instruct patients, families, and home health aides on therapy phases, prosthetic use, and durable medical equipment to promote independence. Compensation ranges from $63 to $105 per visit with mileage reimbursement. Requires an active Michigan Physical Therapy license and reliable transportation.
