Home Health Physical Therapist (PT) - Contingent

Provide skilled physical therapy in accordance with the physician's plan of care, accepting only patients referred by the patient care manager or designee. Assist the physician in assessing the patient's functional level by applying appropriate tests, developing and revising the plan of care, including OASIS assessments and goal setting. Treat patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the plan of care. Observe, record, and report the patient's reaction to treatment and any changes in condition to the physician and/or patient care manager. Instruct and advise the patient, family, and other health team personnel in the phases of physical therapy, including the use of prosthetic and orthotic devices and durable medical equipment.
