Home Health Physical Therapist (PT) - Contingent

Provide skilled physical therapy in accordance with the physician's plan of care, accepting only patients referred by the patient care manager. Assist the physician in assessing functional levels, developing plans of care, and conducting OASIS assessments. Treat patients to relieve pain and restore function using appropriate modalities, while observing and reporting changes in condition to the physician or care manager. Instruct patients, families, and home health aides on therapy phases, prosthetic use, and durable medical equipment to promote independence. This contingent role offers flexible hours with an opportunity to transition to a full or part-time caseload. Visit rates range from $63 to $105 per visit, plus mileage reimbursement.
