Home Health Physical Therapist (PT) - Contingent

Provide skilled physical therapy in accordance with the physician's plan of care, accepting only patients referred by the patient care manager or designee. Assist the physician in assessing the patient's functional level by applying appropriate tests and developing or revising the plan of care, including OASIS assessments and goal setting. Treat patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate modalities and procedures. Observe, record, and report the patient's reaction to treatment and any changes in condition to the physician and staff. Instruct patients, families, and other health team personnel on the phases of physical therapy, prosthetic use, and durable medical equipment to promote independence. Compensation ranges from $63.00 to $105.00 per visit depending on mileage, with additional pay for discharges, transfers, and in-service hours. This contingent role offers flexible hours with the opportunity to transition to a full-time or part-time caseload.
