Job Summary
Physical Therapist Case Manager (PT CM) for Home Health providing therapy services under a physician’s plan of care, assessing functional status, mobility, ROM, directing treatment, and instructing clients and families. Responsibilities include developing the total plan of care, supervising PT assistants, participating in discharge planning, and coordinating with home health team to identify additional home health services. Requires Minnesota PT license, at least two years of clinical PT experience, and adherence to state Physical Therapy Practice Act; includes mileage reimbursement and a Monday–Friday schedule.
Required Qualifications
- Two years of clinical experience as a physical therapist.
- Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association.
- Current and active Physical Therapist license in the state of Minnesota.
- Verification of COVID vaccine status or receiving an approved exemption.
