Home Health Physical Therapist

Evaluate patients, develop individualized treatment plans, and deliver skilled physical therapy services to improve mobility, strength, balance, and functional independence. Assess patient progress, modify plans as needed, and document evaluations, treatments, and outcomes accurately. Educate patients and caregivers on therapeutic exercises, mobility techniques, and fall prevention while collaborating with physicians and the interdisciplinary care team. Maintain compliance with all state, federal, Medicare, and agency regulations. Requires an active Florida PT license, minimum one year of home health experience, and ability to work independently with a flexible schedule. Valid driver's license and reliable transportation required.
