Home Health Physical Therapist

Evaluate patients and develop individualized treatment plans to improve mobility, strength, balance, and functional independence. Provide physical therapy services in accordance with the established Plan of Care while assessing progress and modifying plans as needed. Document evaluations, treatments, and outcomes accurately, educate patients and caregivers on therapeutic exercises and fall prevention, and collaborate with physicians and the interdisciplinary care team. Maintain compliance with all state, federal, Medicare, and agency regulations. Requires an active Florida PT license, minimum one year of home health experience preferred, and ability to work independently with flexible scheduling. Valid driver's license and reliable transportation required.
