Job Summary
Evaluate patients' physical abilities and functional limitations in a home health setting, then develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain. Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining while educating patients and caregivers on home exercise programs, safety techniques, and adaptive strategies. Monitor patient progress, adjust plans as necessary, conduct home safety assessments, and accurately document evaluations and progress notes using digital platforms. Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists. Requires an active Texas Physical Therapist license, graduate degree, and professional liability insurance.
Required Qualifications
- Active Physical Therapist license in Texas
- Professional liability insurance coverage specific to your Texas Physical Therapy license
- Graduate degree or higher in Physical Therapy from an accredited program
- Strong ability to adapt care approaches to suit a variety of patient personalities and needs
- Up-to-date knowledge of evidence-based treatment techniques and best practices
- Excellent communication skills
- Friendly, professional demeanor when educating and interacting with patients
