Home Health Physical Therapist PRN role providing evaluation, development of personalized therapy plans, execution of evidence-based interventions to restore movement and reduce pain, progress monitoring, patient/family education on safe exercise and independence, and collaboration across disciplines with physicians and care teams. Mentoring of therapy assistants or students and adherence to HCA Healthcare values of excellence, compassion, and integrity. Eligible to operate within the HCA Houston Healthcare at Home network; requires licensure as a Physical Therapist and a valid driver license. The position emphasizes in-home, patient-centered care, goal-oriented rehabilitation, and comprehensive outcomes documentation.
