Job Summary
Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential. Develop and implement individualized care plans in collaboration with physicians and the care team. Provide hands-on therapy to improve mobility, strength, balance, and independence while educating patients and caregivers on treatment plans and safety strategies. Monitor patient progress to adjust treatment plans as needed and support safe transitions of care. Partner closely with nurses, therapists, and physicians to ensure coordinated care, document progress accurately within the EMR, and participate in case conferences. Supervise Physical Therapist Assistants in accordance with state guidelines and provide guidance to support staff.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
