Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Deliver a holistic approach addressing physical, spiritual, emotional, and clinical needs across specialty programs like vestibular rehab and home safety assessments. Complete documentation to meet reimbursement and regulatory requirements for Medicare-compliant care. This role offers PRN flexibility with onsite physician support and interdisciplinary team collaboration within a 110-acre campus serving older adults.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
- This position requires Home Health experience
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
