Job Summary
Deliver physical therapy services in patients' homes as part of CenterWell Home Health. Plan and administer prescribed PT treatments, assess physical strengths, develop and implement rehab programs aligned with physician plans, and supervise PTAs/Home Health Aides with clear instructions. Document observations, interventions, and progress; report to physicians and care teams; participate in discharge planning. Requires a licensed physical therapist with CPR certification and team-collaboration experience; one year of PT experience preferred. Eligible for pay per visit or annual base pay; driving role with TB screening and driver-safety requirements.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
