Job Summary
Home Health Physical Therapist PRN provides patient-centered care in a home health setting. Responsibilities include planning, implementing, and evaluating patient care plans (Plan of Care) based on medical referral and evaluations; conducting initial and ongoing clinical assessments including OASIS timing; collaborating with the interdisciplinary team to coordinate services; instructing patients and families on transfer techniques and use of equipment; ensuring infection control and OSHA compliance; documenting progress notes and orders; updating the Plan of Care in consultation with physicians and care-team members; and delivering compassionate, high-quality care consistent with Compassus values. Required education is a Bachelor of Science in Physical Therapy with an active PT license in the employment state and CPR certification; two years of experience in acute care or rehabilitation is highly preferred. Strong communication skills, English proficiency, and the ability to operate standard therapy equipment are essential.
Required Qualifications
- Bachelor of Science in Physical Therapy required
- Two (2) years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred
- Active and unencumbered Physical Therapist license in state(s) of employment required
- Current CPR certification required
- Ability to read, write, and speak English; strong communication skills
- Ability to perform physical tasks including standing, walking, lifting up to 50 pounds
- Knowledge of OASIS assessments and home health regulations
