Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment onsite in patients' homes to attain highest levels of physical function. Assess and document physical strengths to support physician evaluation, then develop and implement rehabilitation programs consistent with the physician's Plan of Treatment. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation while reporting patient progress and submitting required evaluations to the care team. Maintain current and unrestricted Physical Therapy licensure and CPR certification. Pay ranges from $59.00 to $83.00 per visit/unit.
