Home Health Physical Therapist PRN

Plan and administer prescribed physical therapy treatment onsite in patients' homes to attain the highest level of physical function. Assess and document physical strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide clear written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, clinical manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization through personalized home care.
