Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient team objectives. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, Clinical Manager, and care team while submitting required evaluations, plans, and discharge summaries. This role supports CenterWell Home Health's mission to help patients with chronic conditions or recovering from injury regain independence through personalized, comprehensive home care.
