Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment onsite in patients' homes to attain highest levels of physical function. Assess and document physical strengths to support physician evaluations and guide treatment goals, while developing rehabilitation programs consistent with physician plans. Provide written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Document patient observations, interventions, and evaluations promptly, and report progress to the physician, clinical manager, care team, and family. Submit all required evaluations, plans, progress notes, and discharge summaries. Maintain a valid state driver's license and personal vehicle liability insurance.
