Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patient strengths to support physician evaluation, then develop and implement rehabilitation programs consistent with the physician's Plan of Treatment. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, Clinical Manager, and care team, submitting all required evaluations, plans, and discharge summaries promptly. This role supports CenterWell Home Health's mission to help patients manage chronic conditions or recover from injury, illness, surgery, or hospitalization by regaining independence within the comfort of their homes.
