Job Summary
Plan and administer prescribed physical therapy treatment onsite in patients' homes to attain highest levels of physical function. Assess and document physical strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation while reporting progress to the physician and care team. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. Requires a current unrestricted Physical Therapy license, CPR certification, and valid state driver's license.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid state driver's license
- Personal vehicle liability insurance
