Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals, while developing rehabilitation programs consistent with the physician's Plan of Treatment. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, clinical manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries. This role requires a current and unrestricted Physical Therapy license, CPR certification, and a valid state driver's license with personal vehicle insurance.
