Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals, while developing and implementing rehabilitation programs consistent with physician plans of treatment. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, clinical manager, care team, and family, submitting all required evaluations, plans, progress notes, and discharge summaries promptly. Maintain a valid state driver's license and carry personal vehicle liability insurance as part of the driver safety program.
