Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to guide treatment goals, then develop and implement a rehabilitation program consistent with the physician's Plan of Treatment. Provide clear written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation while reporting patient progress to the physician and care team. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization through personalized, comprehensive home care.
