Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals, while developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment. Provide PTAs and Home Health Aides with clear written care instructions, oversee their performance and documentation, and report patient progress to the physician and care team. Document patient observations, interventions, and evaluations promptly, submitting required evaluations, plans, progress notes, and discharge summaries. This role is part of CenterWell Home Health's personalized home care team, supporting patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
