Home Health Physical Therapist PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with physician Plans of Treatment and patient team objectives. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Document patient observations, interventions, and evaluations promptly, then report progress to the physician, Clinical Manager, care team, and families while submitting all required evaluations, plans, progress notes, and discharge summaries. Requires current and unrestricted Physical Therapy license, CPR certification, and valid state driver's license. This role is patient-facing and includes mandatory TB screening.
