Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals, while developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment. Provide PTAs and Home Health Aides with clear written care instructions, oversee their performance and documentation, and report patient progress to the physician, Clinical Manager, care team, and family. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization through personalized, comprehensive home care.
