Job Summary
Provide home-based physical therapy in patients' homes, plan and administer prescribed PT treatment to attain highest physical function, assess and document strengths to guide treatment goals, develop a rehabilitation program aligned with physician plans, supervise PTAs and Home Health Aides with clear written instructions, document observations and evaluations promptly, and report progress to the physician and care team. Requires collaboration with a healthcare team and ongoing communication with patients and family; TB screening and valid driver's license with appropriate automobile coverage are part of the role. Base pay ranges from $93,000 to $128,000 per year; per-visit compensation ranges from $59-$83 per visit. Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
