Job Summary
Provide in-home physical therapy to patients, planning and administering prescribed PT treatments in patients' homes to improve function. Assess and document physical strengths, develop rehabilitation plans aligned with physician treatment goals, oversee PTAs and Home Health Aides with clear care instructions, document observations and evaluations, and report progress to physicians, the clinical team, patients, and families. Requires an active physical therapy license and CPR certification; one year of PT experience preferred. This role is patient-facing, part of Humana/CenterWell Home Health, and includes TB screening and driver-safety requirements.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
