Job Summary
Provide in-home physical therapy to patients by planning and administering prescribed PT treatment in patients’ homes to improve physical function and independence. Assess and document patients’ physical strengths to guide treatment goals, develop rehabilitation programs aligned with physician plans, and oversee PTAs and Home Health Aides with clear care instructions and documentation. Report patient progress to the physician and care team, and complete evaluations, plans, progress notes, and discharge summaries. Travel to patient homes to deliver care and collaborate with a multidisciplinary team to optimize outcomes.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
