Job Summary
Deliver in-home physical therapy to patients, plan and administer prescribed PT treatment in patients' homes to attain highest level of physical function, assess and document patients' physical strengths to guide treatment goals, develop and implement rehabilitation programs aligned with physician plans, supervise PTAs and Home Health Aides with clear written care instructions, document observations and progress, and report to physicians and care team. Requires current PT license, CPR, and ability to collaborate with a healthcare team; pay range $93,000-$128,000 per year with additional per-visit compensation; role involves patient-facing home health responsibilities and travel as needed. Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
