Job Summary
Provide in-home physical therapy to patients, plan and administer prescribed PT treatment in patients' homes, assess and document physical strengths to guide treatment goals, develop and implement a rehabilitation program aligned with the physician's plan of treatment, supervise and give clear instructions to PTAs and Home Health Aides, document observations and evaluations, report progress to physicians and care team, and participate in TB screening and driver-safety requirements as part of Humana CenterWell Home Health. Requires a current Physical Therapy license and CPR certification; one year of PT experience preferred. Compensation includes base pay (salary range $93,000-$128,000 per year) and pay-per-visit options, with benefits described. Scheduled hours and term are per the role, with patient-facing home visits.
