Home Health Physical Therapist-PRN

Provide in-home physical therapy to patients, planning and administering prescribed PT treatments in patients' homes to improve function. Assess physical strengths to support physician evaluation and guide goals; develop and implement rehabilitation programs aligned with the Plan of Treatment. Oversee and provide clear written care instructions to PTAs and Home Health Aides, supervise documentation, and promptly document patient observations, interventions, and evaluations. Communicate patient progress to physicians, the Clinical Manager, the care team, patient, and family, and submit necessary evaluations, progress notes, and discharge summaries. Collaboration with a multidisciplinary team is required. Must hold a current and unrestricted Physical Therapy license and CPR certification. Preferred: one year of experience as a Physical Therapist. TB screening and driver’s license requirements are part of Humana/CenterWell program guidelines.
