Home Health Physical Therapist-PRN

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess patients' physical strengths to support physician evaluations and guide treatment goals, while developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides, overseeing their performance and documentation. Document patient observations, interventions, and evaluations promptly, reporting progress to the physician, Clinical Manager, and patient families. Submit all required evaluations, plans, progress notes, and discharge summaries. This role supports CenterWell Home Health's mission to help patients regain independence from the comfort of their homes.
