Job Summary
Provide in-home physical therapy to patients, planning and administering prescribed PT treatment to improve function and independence. Assess and document patients' physical strengths to guide treatment goals, develop and implement rehabilitation programs aligned with physician plans, supervise PTAs and Home Health Aides with clear instructions, and promptly document observations, progress, and discharge plans. Report progress to physicians, the Clinical Manager, care team, patient, and family, and coordinate care across the team. TB screening and driver-licensing requirements may apply per program; requires active PT license and CPR certification.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
