Job Summary
As a Home Health Physical Therapist, deliver one-on-one physical therapy services in patients’ homes to improve strength, mobility, and independence. Conduct evaluations, develop and implement individualized care plans, and provide treatment to help patients perform daily living activities (e.g., church, gardening) safely at home. Collaborate with families and care teams to address functional needs, monitor progress, and adjust interventions while emphasizing compassionate, patient-centered care.
Required Qualifications
- Bachelor of Science in Physical Therapy
- Current State License as a Physical Therapist
- Valid Driver’s License
- Current CPR
- negative TB screen
- Hepatitis consent/declination
- Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting
- Knowledge of durable medical equipment
- Good interpersonal skills
