Job Summary
Provide one-on-one in-home physical therapy to patients where they need the most help, helping patients maintain independence in daily living activities by improving strength and mobility. The role includes conducting in-home assessments, developing and implementing treatment plans, using durable medical equipment, and collaborating with caregivers and the broader care team to support patients’ ability to live at home. The position emphasizes compassionate care, inclusivity, and a patient-centered approach as part of the Compassus at Home care model.
Required Qualifications
- Bachelor of Science in Physical Therapy
- Current State License as a Physical Therapist
- Valid Driver’s License
- Current CPR
- negative TB screen
- Hepatitis consent/declination
- Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting
- Knowledge and Skills: Therapy skills as defined as generally accepted standards of practice
